Do you sell the self assessment forms?

It states on page one under the introduction of the manual:

“…The Adult Form may be completed by the individual being evaluated for a self-rating if his or her functional skills are judged to be adequate for providing valid responses to the items…”

This means that if the participant is doing a self-assessment it would be the Adult Form that the participant would use to rate themselves. The self assessment norms of this test were standardised using this form.
