The Evaluation Form consists of a Rating Scale, ideas Matrix, and Intervention Plan. The Rating Scale is a criterion-referenced measure for evaluating reading behaviours that impact student learning.

The ideas Matrix provides a systematic way of linking results of the Rating Scale to interventions. The Intervention Plan is a record-keeping tool used to document (1) the targeted-skills, (2) the interventions tried, and (3) the evaulation.

A CD is also included so that all the reproducible items can be easily printed.
